Neurosurgeon Yuriy Perekopayko explained why the female nervous system functions differently and how this alters our understanding of chronic pain.

Today, medicine is re-evaluating this approach. The idea that women not only describe pain differently but that their bodies genuinely experience it differently is gaining traction. This is precisely why conditions like migraines, autoimmune diseases, and chronic pelvic pain are diagnosed significantly more often in women.

The Key Difference

According to the neurosurgeon, one of the primary distinctions is that in women, chronic pain is much more closely linked to the functioning of the immune system. Certain immune cells can amplify pain signals and maintain their activity even when the initial tissue damage has long since healed. In essence, the body continues to “remember” the pain, even if its original cause has disappeared.

This implies that the mechanisms underlying chronic pain in women can differ substantially from those in men.

Hormonal Influence on Pain Perception

Female sex hormones – estrogen and progesterone – also play a crucial role. They directly interact with the nervous system’s pain pathways, influencing neurotransmitters, inflammatory processes, and the sensitivity of pain receptors.

This is why many women notice that their pain intensifies during specific life stages or cycles, such as menstruation, pregnancy, or menopause. This is not a coincidence or an exaggeration but a biological process that scientists are increasingly studying.

Even a Light Touch Can Feel Painful

Another characteristic highlighted by modern research is so-called central sensitization – a condition where the brain and spinal cord become hypersensitive to any pain signals. Consequently, even stimuli that would typically not cause pain can be perceived as unpleasant or intensely painful.

This is why chronic pain can sometimes persist even without an apparent physical cause.

Why Treatment Hasn’t Always Worked

There is another significant factor being discussed more frequently today. The majority of pain relief medications were developed and tested primarily with male participants. Consequently, the mechanisms of female pain remained inadequately studied for a long time.

As a result, many women received treatments that did not address the biological processes underlying their specific pain. This frequently led to delays in correct diagnosis, suboptimal therapy, and, perhaps most distressingly, a devaluation of patients’ complaints.

When pain persists for years, it gradually reconfigures the nervous system’s function, making it considerably more challenging to treat.

Changes in Medicine

Despite the complexity of the issue, recent discoveries offer grounds for optimism. Scientists are increasingly focused on developing personalized pain treatment methods that account for sex differences. This involves therapies that target not only symptoms but also the immune mechanisms, hormonal specifics, and the unique functioning of the female nervous system. Experts believe that such an approach could represent a turning point in the management of chronic pain.

The history of female pain is not just a medical issue but also a social one. For too long, women have heard that their pain was “in their head,” that they were too sensitive, or simply going through a difficult phase. Modern science is gradually dismantling these stereotypes, confirming that female pain has real biological underpinnings that require distinct study and specialized treatment approaches.
